Learning Goal:
Participants will receive information and resources on eldercare, eldercare law and advanced care planning appropriate to the needs of faith community nurses.
Objectives:
By participating in this activity, nurses will be better able to:
• List core legal documents that every person should have
• Outline the different levels of care in Georgia and how each level is funded
• Identify basic VA benefits
• Describe basic elements and benefits of Medicaid
• Discuss need for proactive Advanced Care Planning
• Identify families/patients who are fearful of death
• Outline strategies to help patients/families progress from fear of death to constructive discussion of final health care decisions
• Utilize resources to assist patients/families in making and communicating final health care decisions
• Describe community service programs to help support the needs of patients who might otherwise die without the benefit of immediate family or friends present
Completion Requirements:
Nurses must attend the entire session to receive 3.0 CNE contact hours
